Shannon’s Bridge - Palliative and advanced care

Shannon’s Bridge is a not-for-profit organisation dedicated to helping the terminally ill live their remaining time well, and, when the time comes, to die in the place that they most want to be -- at home. The organisation links up medical specialists, local GPs, community groups and volunteers to develop an advanced care package for individual patients and their families. It then supports them to be comfortable in their own house during the last chapter of their life.

Shannon’s Bridge also provides community education, supported living equipment lending, carer support and pet therapy.
